New clients. You will be emailed an intake packet after scheduling your appointments. Please complete your intake packet and forms prior to your first visit. If you are having difficulty completing the intake packet please contact the Clinic.

What you will need for your first appointment:

• Two pieces of identification.

• Your insurance information, including insurance card(s).

• The prescription from your doctor.

• Comfortable fitting clothing.

• Completed intake forms downloaded from our website. Or arrive 15 minutes early to complete forms in the clinic.

• Your co-payment.

• Your calendar to schedule future appointments.

• If you are a minor, please bring a parent or guardian.
